About The Position

The Case Manager Aide-Vista Haven is responsible for assisting in the coordination of case management activities alongside the case manager through participation in the process of ORR Case Management services with/for the Office of Refugee Resettlement (ORR). The Case Manager Aide provides trauma-informed, individualized case management support according to ORR and other regulatory requirements. Assists the Case Manager with the coordination of the release to the sponsor and ensures the unaccompanied children’s safety.

Requirements

  • Experience providing support to children and youth who have experienced trauma
  • Residential and youth experience
  • Bilingual Spanish-speaking
  • 2 years previous related experience in case management
  • Knowledge of community resources
  • Ability to communicate effectively with others
  • Excellent organizational and time management skills
  • Excellent work ethic
  • Highly responsible and reliable
  • Detail-oriented
  • Ability to multi-task and work at a fast pace
  • Excellent writing skills
  • Proficient on a computer.

Responsibilities

  • Assist clients in obtaining personal documents such as identification and legal documentation as need
  • Expected to have contact with youth on their caseload at least once a week in conjunction with the case manager of the case to assess and move the case forward toward a timely release
  • Assists in gathering sponsor background checks, and fingerprinting checks
  • Gathers all sponsor documentation
  • Attends and participates in cottage meetings and unit meetings
  • Responsible for documenting case notes for all services rend
  • Accurate and timely completion of assigned ORR documentation related to community support and linkages as per identified ORR objective
  • Assist UC in obtaining additional services needed including but not limited to community linkages, education, specialized groups, and mental health/ health services
  • In place of the case manager, communicates all updates to the clinician and treatment team member
  • In conjunction with the case manager, coordinates discharge planning
  • Utilizes strengths-based, trauma-informed, relationship based, culturally responsive, and client and family-centered approach
  • Weekly updates the Case Management tracker to ensure all services are meeting the deadline
  • Audits case files to ensure compliance with stakeholders and funders
