Health Affiliates Maine has an opening for a Veterans Program Eligibility and Care Coordinator to join their team. This is a 40 hours per week/non-exempt position. Key Roles and Responsibilities: Administrative/Specific Position Duties Process Veteran referrals and confirm Fox eligibility for Veterans, including scheduling initial intake with the Veteran and communicating to the referral sources regarding appointment time and status of referrals. If a referral is not appropriate for the Fox program following up with the client and facilitating referral to another entity or support to meet the needs. Collect information on referral sources and services provided to Veterans for data reporting and present this information as requested. Ensure data in the database and electronic health record are consistent data sets, following up on errors and corrections as needed to reconcile data sets. Assist the SSG Fox Team Lead in providing Fox initial and discharge screenings (6-8 per week). Receive and maintain appropriate training to develop and maintain a high-level of comfort with Veteran suicide risk assessment, screening, and referral steps. Train and provide screenings using the following documentation and standardized intake and assessments which includes at minimum the following instruments: Columbia Suicide Severity Rating Scale, Socio-Economic Status; PHQ-9; Short Warwick Edinburgh Mental Wellbeing Scale, Generalized Self-Efficacy Scale, and Interpersonal Support Evaluation. As requested by the Team Lead, provide discharge screening of the above instruments and other discharge paperwork with all Veterans that exit the SSG Fox program. Build a rapport with Veterans. Provide a monthly check-in call and service satisfaction survey for a group of Veterans assigned by Team Lead. When a Veterans Case Manager is not immediately available to take a Fox case or services are likely to be brief (less than 90 days), they provide Veterans with interim case management services following all policies and procedures of the case management program, including initial intake, assessment, service plan development and provision of services. Be prepared for and actively participate in Department staff meetings and individual supervision. Document in a timely manner in the electronic health record as prescribed by policies and procedures. Enter quality documentation as required in the electronic health record as prescribed by case management policies and procedures. Complete all required Veterans grant reporting in the data collection tool in a timely and thorough manner. Maintain and complete accurate client files accordingly to agency policies, procedures and in compliance with applicable law, licensing regulations, contract agreements, MaineCare regulations, consent decree and the rights of recipients. Provide services in an ethical manner by maintaining professional relationships. Use the NASW (National Association of Social Workers) Code of Ethics, MHRT/C code of conduct, applicable laws, and policies to help resolve ethical dilemmas. Maintain positive and collaborative relationships with all community partners. Protect and maintain the confidentiality of client records and information, following HIPAA, HITECH regulations and 42 CFR. Practice a trauma-informed approach to care including maintaining a calm, non-defensive attitude to de-escalate crisis situations, encouraging the health, safety, and welfare of clients, and ensuring self-determination. Participate in mandated reporting per agency policy and Maine Law including the review and submission of critical incidents, reportable events, and DA reports, Perform other duties related to the position as directed by the Leadership Team.
