This position focuses on housing placement and sustainability, participant engagement and socialization within the community, to prepare participants for self-sufficiency post subsidy. The right person for this role is someone who has creative problem-solving skills, is highly engaging and organized with strong motivational skills to help participants stay on track to accomplish their housing stability goals. This is a non-exempt, represented full-time position. The Transition Projects Case Management program covers three distinct areas of participant service: Access, Housing and Retention. Employees hired as Case Managers are expected to be able and willing to perform the duties of each of these functions and may have duty priorities adjusted as needed. Duties may take place at any of our service sites or off premises.
