About The Position

Responsible for the incident reporting function of the Delta Residential Division, and other locations as needed, including entering all reportable incidents, conducting certified investigations and generating reports for data analysis. Advances Delta’s mission and values.

Requirements

  • Ability to read, write and speak English; perform arithmetical calculations.

  • Proficiency in computer programs such as Word, Excel and database administration.
  • Ability to operate vehicles with a valid driver’s license in accordance with applicable state laws, insurance provider requirements and company policies and procedures.
  • Bachelor’s degree from an accredited college or university
  • Two (2) or more year’s work experience working with incident management systems and community services to people with disabilities.
  • Strong analytical writing and presentation skills.

Responsibilities

  • Act as the Point Person receiving verbal and/or reports of incidents, ensuring the appropriate actions have been taken or order additional actions to secure the health and safety of the individual(s) involved in the incident.
  • Lead and/or assign investigators as needed for each incident.
  • Ensure that all reportable incidents are submitted electronically via the web-based system, within a timely manner. This also includes, at times, requiring work on weekends and holidays.
  • Act as a contact with county and regional office risk managers on routine incidents.
  • Collect all written incident reports for data analysis and incident management; organize incident data in a way that is useful for supervisors to protect individuals from harm.
  • Develop quality indicators and strategies based on trend analysis to reduce the occurrence of incidents an work with residential Managers to implement strategies.
  • Monitor the implementation and measure the effectiveness of strategies.
  • Generate routine reports to be used in day to day organization; incident analysis, Bi-monthly Risk Management Meeting Notes and Reportable and Non-Reportable Monthly Summary Reports.
  • Analyze and present reports in Risk Management and other meetings as required.
  • Act as Administrator to the online data management systems.
  • Provide staff with the technical assistance needed in using these web-based systems.
  • Responsible for coordinating and assisting with Provider Quality Assurance and Improvement Process.
  • Responsible for assisting with regulatory compliance oversight; maintains current knowledge of laws and regulations relevant to community living services.
  • Responsible for assisting with quality management processes.
  • Participate in agency, departmental and program meetings as required.
  • Participate in Delta’s Continuous Quality Improvement efforts through active committee involvement.
  • Assist in training for employees in incident management reporting requirements and incident prevention.
  • Promote and demonstrate ethical and professional conduct at all times ensuring employees conduct themselves in an ethical and professional manner.
  • Promote continuous quality improvement projects and ongoing performance measures.
  • Promote and address Delta’s Risk Management Program for the health and safety of individuals and employees.
  • Perform such other duties that may be assigned for providing excellent customer service and for the most effective operation of the company.
