About The Position

Visiting Nurses Association (VNA) an affiliate of Midland Care Connection has been providing trusted care for over 130 years. We’re looking for dedicated nurses who thrive in independent roles, have strong critical thinking skills, and want to make a meaningful impact by supporting patients and families during evenings, weekends, and after hours. After Hours/Weekend Nurse responds to patient, family, caregiver, and referral source calls. This role requires excellent clinical judgment, independent decision-making, and the ability to deliver quality nursing care in a home and community-based setting. You will work closely with the interdisciplinary care team, ensuring continuity of care while maintaining high standards of nursing practice.

Requirements

  • Graduate of an accredited school of nursing (RN).
  • 1–2 years of recent acute care experience, plus 1–2 years of home care experience preferred.
  • Current licensure in Kansas and Missouri and CPR certification.
  • Licensed driver with a reliable, insured automobile.
  • Strong communication, assessment, and problem-solving skills.
  • Ability to lift, transfer, and reposition patients; prolonged standing and walking required.
  • Proficiency with electronic charting and documentation.

Responsibilities

  • Provide timely response to calls from patients, families, and triage/answering service.
  • Assess patient needs and deliver appropriate interventions by phone or in-person visits.
  • Implement changes to the plan of care in collaboration with physicians and the care team.
  • Document all assessments, interventions, and communications according to policy.
  • Provide emotional support and education to patients and families.
  • Attend patient deaths to support families, coordinate with mortuaries, and ensure compliance with protocols.
  • Perform blood draws, administer medications, and deliver skilled nursing care per physician orders.
  • Serve as case manager for assigned patients when delegated.
  • Supervise and support nursing aides when needed.
  • Participate in on-call duties, staff meetings, and quality improvement initiatives
