The Specialist is responsible for travel to patients with gaps in chronic disease management and preventive care. This role serves to support health status improvement through access to services, education, wellness support and reinforcing the importance of the patient/primary care provider relationship. Some of the preventative screenings being done are diabetic eye exams and other preventative screenings. Travel would be required up to three days per week, with mileage reimbursed at .65 cents per mile. Overtime would be available. Coverage area will include multiple counties in the southeast region of Michigan.
