In this role, your responsibilities center on delivering safe, high-quality patient care for older adults who require ongoing assistance with daily living activities, chronic disease management, and rehabilitation goals. You’ll perform essential nursing tasks within the LPN scope, including monitoring vital signs, administering medications as prescribed, assisting with activities of daily living, wound care, skin integrity assessments, and timely documentation in the patient chart. You’ll collaborate closely with registered nurses, physicians, and interdisciplinary teams to implement individualized care plans, evaluate progress, and adjust interventions as needed. You’ll play a key role in fall prevention, infection control, and mobility support, while offering education and reassurance to residents and families.
