What you can expect as a Transition Coordinator We are seeking a dedicated Transition Coordinator to support students and families as they prepare for life beyond Perkins. Serving students in the Secondary program, this role ensures a smooth, well-planned transition to postsecondary opportunities. You will play a key role in coordinating transition planning, connecting students with adult service agencies, and fostering strong self-determination skills.

Requirements

  • Bachelor’s degree in Special Education or a related field required.
  • Minimum of 3 years’ experience in special education.
  • Transition Specialist Certificate and knowledge of Turning 22 processes required.
  • Strong ability to collaborate with students, families, colleagues, and adult service providers.
  • Effective communication, problem-solving, and teamwork skills.
  • Ability to follow instructions, contribute to planning, and support the broader instructional team.

Nice To Haves

  • DESE or CEC/DCDT certification recommended.

Responsibilities

  • Collaborate closely with the Director and Assistant Director of Transition Services, Case Managers, and clinical team members to deliver effective transition planning.
  • Participate in IEP meetings, case reviews, and planning sessions for students age 14 and older.
  • Promote active and meaningful student involvement in all phases of transition planning, ensuring students of all abilities can express their interests, preferences, and goals.
  • Work with families, Case Managers, and IEP teams to develop transition components of the IEP, including considerations for turning 18, long-term goals, diploma requirements, and service alignment.
  • Support age-appropriate transition assessments, compile Transition Portfolios, and coordinate Person-Centered Planning with educators and residential staff.
  • Schedule, facilitate, and document Quarterly Transition Meetings with Case Managers and clinicians.
  • Build and maintain collaborative relationships with adult service agencies and community partners.
  • Provide transition counseling to families on topics such as adult services, age of majority, program exploration, and community resources.
  • Assist families in arranging visits and tours of potential postsecondary or adult service programs to support informed decision-making.
  • Offer ongoing support to students and families navigating adult services, maintain contact post-graduation as appropriate, and oversee data tracking related to postsecondary outcomes.
  • Maintain accurate records of transition activities, data, and outcomes.
  • Teach or co-teach transition-related classes using established curriculum; develop and report on measurable IEP transition goals.
  • Provide transition-related in-service training to staff, including IDEA and transition-aligned IEP requirements.
  • Share community resources with students and families; assist with the Transition Symposium and related events.
  • Contribute transition updates and information for the Family Newsletter.
  • Attend meetings with disability advocacy groups to stay current with transition practices and resources.
  • Perform other duties as assigned.
