The Transition Assistant (TA) Manager, under the direction of the Administrative Director of Care Management, will be part of the Population Health regional management team. In alignment with the strategic and operational goals of Hackensack Meridian Health (HMH), this position is responsible for providing leadership, planning and organization, as well as monitoring the outcomes of the Transition Assistant team across multiple regions. Responsible for the development of guidelines and protocols which promote the development and and implementation of a comprehensive program to achieve desired outcome measures. This position will identify improvement opportunities and facilitate process standardization across regions to maximize resources and promote organizational efficiencies. Must understand the clinical and financial aspects of Population Health and be comfortable working with all levels of management and staff. As an essential element to the overall success of the ACO, this position will consistently demonstrate a strong work ethic and commitment to excellence.

Requirements

  • High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
  • Minimum of five or more years of recent ACO, Population Health or other value based program experience.
  • Minimum of four or more years in a community medical office setting.
  • Minimum of three or more years in a supervision role.
  • Excellent analytical skills.
  • Good reasoning and problem solving ability.
  • Highly developed interpersonal skills.
  • Effective teaching techniques for multiple levels of staff and management.
  • Proficient at multitasking and prioritization.
  • Ability to work independently as well as in a team environment.
  • Highly organized.
  • Good presentation skills.
  • Demonstrated proficiency with Google, PC and Microsoft office skills. Ability to learn and become proficient in Population Health electronic medical records and software applications.
  • Required travel time to local northern New Jersey practice is approximately 50%.
  • Excellent written and verbal communication skills.

Nice To Haves

  • Bachelor's degree.
  • Minimum of 3+ years of medicare and commercial payer auditing experience.
  • Bilingual in Spanish.
  • Prior leadership and Clinical experience.

Responsibilities

  • Responsible for day-to-day management of the Transition Assistant team in multiple HMHN regions.
  • Responsible for the oversight of the Transitions Assistant Supervisor for the Central region. 2, Develops, implements and routinely revises metric workflows for input and output management to ensure consistency among regions.
  • Direct responsibility for metric collection and documentation processes of TA staff in over 50 off-site physician offices.
  • Evaluates productivity weekly and monthly to ensure individual and group performance standards are met both in quality and quantity of work.
  • Facilitates formal and informal in-services and training for staff education.
  • Monitors performance of staff and adherence to established workflows, procedures and policies and provides constructive feedback and coaching.
  • Provides remote and on-site support to the Transition Assistants and other team members.
  • Work closely with the Administrative Director to address open issues and consistently suggest workflow enhancements.
  • Collaborates with other leaders of the Population Health team to develop policies and procedures to support systems, standards, and evidence-based practices.
  • Develops Transition Assistants tools needed to streamline patient care and reporting.
  • Works closely with Clinical ACO Manager on Population Health improvement strategies (e.g., systems and policy advocacy, program or policy development, or other community-based interventions).
  • Interviews and hires new team members.
  • Conducts regular Team Meetings with and without the Director.
  • Completes miscellaneous projects and other responsibilities as assigned by the Administrative Director.
  • Updates and educates the Transition Assistance Team on new responsibilities and objectives.
  • Review/analyze multiple sources of data on a weekly and monthly basis to ensure department workflows are in place.
  • Works closely with the regional practices to communicate responsibilities and increase their engagement as well as ensure the practices satisfaction with the services provided by the Transition Assistant Team.
  • Notifies the Director of any concerns related to the Transition Assistant team.
  • Other duties and/or projects as assigned.
