About The Position

The Population Health Assistant will support the planning, execution, and evaluation of population health initiatives. This role focuses on data preparation, reporting, care gap tracking, program logistics, and cross-team coordination to improve quality metrics, address social determinants of health (SDOH), and support regulatory and value-based care performance. Requirements

  • Associate’s degree in Health Administration, Public Health, or related field required.
  • Minimum of 2 years of experience in a behavioral health environment.
  • High attention to detail and strong organization skills are essential. Experience using electronic health care records.
  • Excellent interpersonal skills, capable of confidently and effectively presenting and speaking.
  • Able to work independently, collaboratively and proactively to accomplish work goals.
  • Proficient in Microsoft Office Suite and managing projects through a full cycle process.
  • Proficient in basic date analysis, reporting to include spreadsheets, charts and diagrams.
  • If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required. At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
  • Must have a Fingerprint clearance card or be eligible to obtain one.

Responsibilities

  • Pull, clean, and validate data from EHR, registries, and quality/analytics platforms to track care gaps, risk stratification, and program performance.
  • Produce ad-hoc reports (e.g., HEDIS measures, SDOH indicators) for leadership and clinical teams.
  • Maintain accurate documentation of data sources, refresh cycles, and report definitions.
  • Coordinate logistics for population health projects (scheduling internal meetings, tracking action items, maintaining project plans).
  • Prepare materials/presentations for internal huddles/committees/leadership—agenda, minutes, metrics summaries, and issue logs.
  • Support clinical and care management teams with roster updates, panel attribution lists, and internal workflow documents.
  • Process internal referrals from providers or care teams (e.g., SDOH, behavioral health) and ensure proper documentation and hand-offs—no direct member contact.
  • Assist with quality improvement cycles (PDSA), audit readiness, and documentation for accreditation or regulatory programs.
  • Track measure definitions, changes, and deadlines; ensure internal documentation is current and accessible.
  • Support internal policy/procedure updates and standard operating procedures related to population health workflows.
  • Maintain user guides, tip sheets, and process maps for internal tools (EHR registries, data exports, BI dashboards).
  • Triage internal data/technical issues and escalate to IT/analytics as needed; track resolutions.
  • Coordinate with care management, quality, analytics, and provider relations teams to share internal updates on care gap status, workflow changes, and deadlines.
