Key member of the practice team who has an understanding of primary care practices and assists in the coordination of care required for optimal coordinated population health. Requirements

  • High school graduate or GED equivalent preferred
  • More than one year of related experience in a medical setting
  • Basic computer literacy
  • Knowledge of medical terminology
  • Good communication and customer service skills
  • Able to perform all Medical Office Assistant job functions and responsibilities
  • Valid NYS drivers’ license

Nice To Haves

  • Current Medical Assistant certification or registration preferred

Responsibilities

  • Documents all patient interactions in a concise manner that is compliant with documentation requirements
  • Conducts in-bound and out-bound calls including, but not limited to: patient scheduling, screenings, reminder calls, panel management
  • Assists to schedule and coordinate patient transportation, follow-up physician appointments
  • Utilizes various reports to track patient referrals and scheduling no-shows for patients in collaboration with practice staff
  • Provides clerical and / or administrative support to clinical staff and managers for special studies, projects and reports
  • Provides excellent customer service by serving as a resource to all internal and external customers
  • Maintains knowledge of all health plan benefits and payor requirements,
  • Call patients as designated by PCP or clinical staff
  • Verify patients have been seen and follow up on no show patients
  • Assist all patients with barriers
  • Ensure appropriate clinical information is communicated to clinical staff when there is variance from plan of care
  • Coordinates other medical office assistants in outreach and report validation needs
  • Closes Care Gaps
