About The Position

Under general supervision, promotes access to mainstream benefits programs for eligible individuals including but not limited to: SSI/SSDI; Medicaid; SNAP; Vocational Rehab, and other benefits programs based upon assessed need of the program participant. This position will also provide a broad range of paraprofessional social services and case management duties to individuals and families receiving services at the Homeless Day Service Center. Conducts program intakes and eligibility assessments, participates in treatment team planning, and coordinates follow up with mainstream community partners. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. Additional duties that are unique to HDSC include – management of multiple intake processes to include GPRA, HMIS, CCBHC NOMS, and Coordinated Entry process. This position will also provide coordination and supervision of other employees as applicable while working with individuals navigating access to mainstream benefits.

Requirements

  • Bachelor’s degree in a social service-related field from an Accredited college or university OR High school diploma or GED AND Two years of experience in social services related position OR One year of experience at the lower level Social Svcs Tech 1 (SST010) or position equivalent.

Nice To Haves

  • Experience working with mainstream benefits organizations such as Disability Adjudication Services, Social Security Administration, DFCS, Dept. of Labor, etc. Must demonstrate knowledge of eligibility determination and referral process associated with mainstream benefits systems.
  • Associate’s level degree in Social Services or other related field. Proven experience can be substituted for formal education. Individuals with lived experience of homelessness are strongly encouraged to apply. A passion for connecting with individuals with behavioral health barriers is a must.
  • At least 1 year of related experience and/or training pertaining to mental health service provision, working with unhoused individuals and their families, community or residential-based services, certified peer specialist certifications, or other relevant areas.

Responsibilities

  • Maintain knowledge and capacity to refer and connect eligible program participants to mainstream benefit programs
  • Screen and assess individuals to determine eligibility for mainstream benefits programs
  • Coordinate and promote access to mainstream benefit programs based upon determined eligibility
  • GPRA, HMIS, NOMS, Coordinated Entry Intake and case load management to ensure adherence to grant and community level guidelines
  • Coordinate, support, and participate in monthly internal fidelity reviews and program level audits to ensure compliance with grant funding program sources.
  • Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services
  • Ongoing review of client progress and service note updates to document client progress toward stability
  • May counsel clients within the community or clinic setting and provide skill building services to assist with financial and social skills
  • Implements and organizes the delivery of specific social services within the community
  • Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress
  • Coordinates HMIS system data entry to maintain compliance with data requirements in ESG-funded program.
  • Maintains program statistics for purposes of evaluation and research
  • Monitors and documents progress towards accessing eligible mainstream benefit programs
  • Prepares intake reports and case history records
