This position is for a Therapist specializing in Mental Health Care Intake. The therapist will conduct adult intake assessments, develop treatment plans, and update treatment plans for adults with severe mental illness. These intakes will occur in an outpatient Community Mental Health Center. The role offers a valuable opportunity to gain experience in diagnosing, crisis intervention, and collaborative care, and involves making necessary internal and external referrals. Family & Children’s Services is dedicated to supporting children, strengthening families, and aiding individuals in recovery from mental illness and addiction. The therapist will operate within an innovative and dynamic Certified Community Behavioral Health Clinic (CCBHC) model of care. This transformative, team-based approach focuses on whole-person care for both adults and children facing complex mental health and substance use challenges. Multi-disciplinary teams collaborate to ensure coordinated efforts for enhanced client recovery. The CCBHC model aims to improve client outcomes and quality of care through increased access to crisis services, expanded community mental health and substance use services, integrated care coordination with physical health screenings, greater access to social services, and specialized support for veterans and those affected by the opioid crisis.
