About The Position

The role of the Team Leader is to supervise and coordinate the case management services provided by teams of case managers to adults with severe and persistent mental illness and/or children with severe emotional disturbance.

Requirements

  • Bachelor's degree in social work/psychology or human services related field
  • Two years case management experience of which at least one year is experience in community based mental health with adults recovering from mental illness OR combination of equivalent experience and training

Responsibilities

  • Supervise a team of case managers to ensure the effective delivery of service, facilitate team meetings, collaborate with clinical supervisors to provide ongoing coaching, mentoring and supervision in an atmosphere that fosters open communication to ensure case managers are supported in their roles and understand performance expectations
  • Coordinate work schedules to ensure coverage of program needs, communicate openly, directly and professionally with all team members
  • Ensure that staff complete CEU requirements, Rule 79, county and MHR requirements
  • Collaborates with Human Resources and complies with MHR policies and procedures with staff recruitment, orientation, corrective action plans, retention, and training of case managers
  • Review and evaluate the performance of case managers, including performance appraisals in accordance with MHR policies and procedures
  • Assist in the development and implementation of programs goals and objectives
  • Attend and participate in all programs and Agency staff meeting
  • Ensure timely submission of all civil commitment paperwork
  • Complete assigned reports and ensure collection of required data, audit case records, reports, and paperwork to ensure it meets agency & rule 79 requirements
  • Monitor teams to ensure services to clients are meeting agency expectations and all documentation is in accordance with MHR documentation standards and Rule 79 requirements
  • Conduct client intake assessment and assign cases to team
  • Be responsive to clients’ and stakeholders’ concerns when quality of care is compromised by direct staff or agency practices
  • Assess and monitor services rendered to clients
