About The Position

This is a full-time non-exempt position with primary working hours of Monday through Friday 8:30am-4:30pm (37.5 hours/week) located in Greensburg, PA. The role offers a guaranteed hourly rate increase at 12 months of service and paid training. A Targeted Case Manager provides specialized case management services, including educating clients on available resources, assisting with goals for basic living needs (housing, food, medical care, recreation, education, employment), and may accompany clients to appointments. The position involves liaising with therapists, psychiatrists, in-home therapeutic service providers, and approved private school placements for adolescents, as well as linking with Children and Youth agencies and Juvenile Justice Programs. Specialized case management services are provided in geriatric, forensic, and Mental Illness and Substance Abuse (MISA) areas, with direct involvement in discharge planning from state and community hospitals. There is an occasional, rotating on-call shift for after-hours crisis situations.

Requirements

  • A bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, criminal justice, or other related social science
  • OR an associate degree with 2 years’ experience in human services or related field and 12 credits in social sciences
  • OR a high school diploma with 2 years’ experience in human services or related field and 12 credits in social sciences

Responsibilities

  • May accompany the client on appointments related to their physical and behavioral health.
  • Liaison with the client’s therapist and/or psychiatrist, in-home therapeutic service provider or approved private school placements for adolescents.
  • Link with Children and Youth agencies and Juvenile Justice Programs.
