About The Position

This is a full-time non-exempt position primarily working Monday through Friday from 8:30am-4:30pm (37.5 hours/week). The role involves assessing client functioning and developing comprehensive Targeted Case Management (TCM) plans for individuals transitioning from psychiatric facilities. Key responsibilities include coordinating discharge planning, linking clients to essential services like housing and benefits, and providing intensive monitoring during the initial 90 days post-discharge. The position also requires overseeing all client services, ensuring needs are met through community supports, attending interdisciplinary meetings, and maintaining a current directory of resources. A critical component is participation in a rotating 24-hour crisis response system, offering immediate support and exploring alternatives to hospitalization. The Acute Targeted Case Manager acts as a strong advocate for clients, addressing barriers, resolving issues, and ensuring access to appropriate services and resources, while also maintaining accurate records, ensuring regulatory compliance, and preparing for audits.

Requirements

  • Master’s degree in social work, psychology, rehabilitation, activity therapies, counseling; OR
  • Bachelor’s Degree which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science AND three years of direct experience in behavioral health, of which two years must be experience working with or providing services to adult individuals with serious mental illness or to children with serious emotional disturbance or at risk for same; OR
  • Bachelor’s degree in nursing and 3 years of direct mental health care experience.

Responsibilities
