About The Position

This position will develop, implement, and direct the daily functioning and staffing needs of the Transitions Across Community Team (TACT) Program. The TACT Program Manager will assume responsibility for process and performance improvement activities as they relate to nursing and ancillary personnel and assist with community education. Requirements

  • Effective leadership and professional skills according to Standards of Care.
  • Ability to assess a patient’s condition, formulate plan of care, select appropriate interventions, evaluate patient’s response to plan of care and to explain to patient/family about this process
  • Ability to assess, evaluate, explain, teach, problem solve using critical thinking; and perform calculations
  • Ability to work independently while collaborating with other team members
  • Ability to train staff to emulate a care transitions model in order to have expected outcomes
  • Knowledge and appreciation of cultural diversity and low literacy issues, and how those factors can affect coaching and health care provision
  • Knowledge of hospital structure, nursing policy and procedures, and chronic disease management
  • Ability to drive to and from a variety of settings in varying weather conditions
  • Bachelor’s Degree or higher in Nursing or Social Work related field
  • Wyoming Registered Nurse (RN) License or enhanced Nurse Licensure (eNLC) • OR, a PCSW or LCSW license issued by the Wyoming Mental Health Professions Licensing Board
  • Three (3) or more years of experience in a healthcare related field
  • Valid USA driver’s license
  • Must have clean driving record that follows Cheyenne Regional Medical Center’s driving guidelines
  • Cheyenne Regional AHA RQI within 14 calendar days of start date

  • Case Management, Social Work, Home Health Care or Population Health Experience

Responsibilities

  • Responsible for the operational and logistical aspects of the Transitions Across Community Team Program.
  • Provides leadership, guidance, and support to direct reports.
  • Ensures that scheduling provides appropriate staff coverage for the department. Assure that the workload is utilized appropriately.
  • Interviews, hires, and supervises/facilitates orientations for all new employees.
  • Coordinates management across the continuum of TACT cares, to include planning and implementation of clinical protocols and practice management guidelines, monitoring care of patients, providing direct patient care and serving as a resource for clinical practice.
  • Provides professional staff development and oversight, participate in case review, implement practice guidelines, and direct community education.
  • Serves as a liaison to leadership and represent the TACT program to various hospital and community committees.
  • Designs the collection tools to facilitate performance improvement activities, trend reports, and research. Facilitates protocol design for accurate data collection, feedback, and analysis.
  • Works collaboratively with physicians, hospital and clinical staff to track progress of patients and identify opportunities to assist in improving quality outcomes. Monitors patient’s response to treatment, works with patient’s team to analyze and address unanticipated developments, and assists in removing barriers to care.
  • Provides continuity of care management for those patients with multiple chronic conditions.
  • Provides accurate, on-going documentation of care coordination activities to include care oversight, case management and discharge planning needs.
  • Develops standardized care plans with PCMH/PCP/Specialty providers; including detailed discharge orders, medication instructions and follow-up care.
  • Monitors monthly financial reports to ensure the unit remains within established budget.
  • Provides operational leadership and has budget responsibilities.
