About The Position

Responsible for assisting with the delivery of community support services for individuals receiving services through our Clustered Apartments Program; coordination of delivery of mental health services; resource development and linkage with community; functions as a professional staff member of a multi-disciplinary team providing comprehensive assessment and treatment services to clients. Clustered Apartments allow for on-site monitoring and on-site interventions and treatment of individuals who require an intensive level of care and support, and who may require skills in the necessities of daily living. Clustered Apartments provide low income subsidized housing assistance to persons with disabilities, mental illnesses and/or poverty. Community Counseling Center (CCC) - Your Behavioral Health + Wellness Resource - is a behavioral health organization that has been serving the public since 1974. We are an administrative agent for the Missouri Department of Mental Health, and we are entrusted to provide a full array of comprehensive behavioral health services within our five county catchment area.

Requirements

  • HS Diploma/HSE/GED is required.
  • Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance. Must also meet CCC's Motor Vehicle Record standards policy and have reliable transportation.
  • Due to this position requiring staff transportation of clients in extenuating circumstances, this position also requires: reliable transportation, valid driver's license, and proof of continuous auto insurance.

Nice To Haves

  • Two years of mental health or direct health care experience preferred.

Responsibilities

  • Provide training to clients in daily living skills.
  • Facilitate development and/or enhancement of personal support systems within the client's community.
  • Assist clients in creating and participating in recreational activities as indicated or available.
  • Perform bimonthly apartment inspections and provide input/skills training on household management.
  • Maintain work productivity standards as established by supervisor and in accordance with assigned caseloads.
  • Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
