The Program Clustered Apartments allow for on-site monitoring and on-site interventions and treatment of individuals who require an intensive level of care and support, and who may require skills in the necessities of daily living. Clustered Apartments provide low income subsidized housing assistance to persons with disabilities, mental illnesses and/or poverty. The Job Responsible for assisting with the delivery of community support services for individuals receiving services through our Clustered Apartments Program; coordination of delivery of mental health services; resource development and linkage with community; functions as a professional staff member of a multi-disciplinary team providing comprehensive assessment and treatment services to clients.
