The Supportive Housing Clinician/Case Manager works with adult clients who have a mental illness/chemical dependency and assists them in becoming as independent as possible in the community. The clients are involved in multiple systems and through the provision of a full range of case management services, supportive housing and service coordination, the Clinician/Case Manager assists clients in on-site setting and assists them in attaining short and long term goals. This role will work closely with Housing First. Location: 4238 Auburn Way N Auburn, Washington 98002 United States Schedule: Monday-Friday 8AM- 5PM Responsibilities GENERAL SUMMARY The Clinician/Case Manager works with adult clients who have a mental illness/chemical dependency and assists them in becoming as independent as possible in the community. The clients are involved in multiple systems and through the provision of a full range of case management services, supportive housing and service coordination, the Clinician/Case Manager assists clients in on-site setting and assists them in attaining short and long term goals.

Requirements

  • A minimum of a Bachelor's degree in social work, psychology, or a counseling-related field, with a strong emphasis on working with families and community agencies.
  • A minimum of one year of clinical experience as a clinician or case manager.
  • Demonstrated skills in clinical assessment and treatment plans, required.
  • Skill and experience in working with: A wide range of emotional and behavioral problems in adults;
  • Understands the principles of individual therapy, group therapy and/or case management approach to providing services.
  • Understands recovery concepts in behavioral health; synthesizes and applies, as appropriate to their work at Sound.
  • Ability to learn new tasks and skills quickly, and to manage and prioritize multiple tasks, while simultaneously managing crises.
  • Able to demonstrate the capacity to appreciate and work well with the complex and diverse population.
  • Washington driver’s license and insurable driving record required.
  • Driving is an essential function of this position. The clinician will conduct outreach in the community, which requires driving between various locations in King County.
  • Must be able to obtain a WA State Agency Affiliated Counselor credential within the first 60 days of hire.
  • be able to sit at a desk for four or more hours a day, and use office equipment, including phones and computer keyboards, for four or more hours per day.
  • Individuals must be able to engage vendors, team members, clients in person and over the phone by voice.
  • Regular attendance is an essential function of the Job.

Responsibilities

  • Provide, direct services and/or internal and external organization referrals for clients.
  • Develop treatment plans and maintains accurate and timely clinical records according to organization policy.
  • Provide housing search and housing stabilization services to clients who are homeless.
  • Provide on-site services at the housing site during their scheduled work hours.
  • Assess level of functioning, developing (and updating) comprehensive, insightful treatment plans with long and short term goals based on level of functioning, outcome measures and client needs
  • Provide crisis intervention services as necessary;
  • Provide counseling, individual treatment services and family involvement as necessary;
  • Assist clients to remain stable in the community, thereby reducing the use of hospitals or involuntary treatment services;
  • Inquire and monitor medication regimes and treatment compliance;
  • Teach problem-solving, goal-setting, social skills, communication skills, money management and accessing community resources;
  • Monitor utilization of services and the success of outcome measures.
  • To comply with county and state standards there will be service intensity expectations for this position, if assigned.
  • Maintain working relationships with community and government agencies, other professionals, family members of clients and significant others.
  • Complete all documentation in timely manner.
  • Meet agency productivity standards.
  • Handle client grievances and implement the client grievance procedure on behalf of the client.
  • Maintain ethical and competent client and peer relationships in the delivery of service.
  • Comply with all applicable federal, state and county laws and regulations, Sound policies and procedures and all safety regulations.
  • Participate in team member meetings, consultations, case presentations and in-service training.
  • Participate in ongoing program development and assessment.
  • Performs other duties as assigned.
  • Establishes and maintains positive, ethical, and professional working relationships with clinical team members, clients, and outside organizations.
  • Relates to a wide variety of people including members of ethnic and sexual minority populations, and is respectful of individual and cultural differences.
