BHcare is seeking a Supportive Housing Case Manager to join our team. This role is focused on helping families build safe, stable, and nurturing home environments while working toward reunification and long-term independence. What You’ll Do: • Complete intakes and assessments to evaluate eligibility for services. • Develop, monitor, and update individualized care plans. • Provide advocacy, crisis intervention, and referrals for counseling, substance abuse recovery, parenting, childcare, education, employment, and other support services. • Coordinate with DCF social workers, treatment providers, and partner agencies to ensure family safety and stability. • Conduct home visits (including unannounced visits) and provide transportation when needed. • Maintain accurate case records, data entry, and prepare monthly reports. • Participate in team meetings, supervision, and professional development. • Support program development and community outreach efforts. What We’re Looking For: • Strong case management skills with experience supporting families. • Ability to coordinate multiple services and work collaboratively with partners. • Strong organizational, communication, and advocacy skills. • Flexibility to work evenings or weekends as needed. • Reliable transportation (use of personal vehicle required).
