OVERVIEW OF PRIMARY RESPONSIBILITES: Provide case management services to assigned participants, including service planning, supportive counseling, and required home or office visits. Coordinate care with the Senior Case Manager and interdisciplinary team, assist participants with housing placement, entitlements, and community referrals, and maintain accurate documentation in accordance with program requirements. Travel Requirements: This job can require up to or more than 10 hours per week on a full-time basis, depending on the need of the clients, in which is equivalent to 30% travel expectations per week
