About The Position

This position is responsible for the day-to-day operations of HUD Scattered Site Housing Program, OASAS Permanent Supportive Housing Program, Shelter Plus Care Housing Program, and Garden Lofts at Marine Hospital. This position is responsible for all aspects of case management for individuals diagnosed with substance use disorders who may be experiencing homelessness. Duties and Responsibilities Assists program participants in obtaining housing that meets standards of quality set by HUD Supports program participants in selecting household furnishings Assists program participants during the “moving in” process Facilitates relationships between program participants and landlords/property managers, advocating for program participant when necessary Establishes working relationships with landlords/property managers and maintains a roster of available housing units Works in collaboration with program participants to create individual service plans to support participant’s growth, empowerment, and self-sufficiency Informs appropriate staff of any concerns or changes in program participant’s demeanor that may signify exacerbated mental health concerns that require intervention. Visits program participant’s homes at least once monthly to inspect the unit and ensure participant’s safety and security in the home Assists program participants in securing linkages to social supports that lead to successful community integration Assists program participants in achieving financial independence by obtaining income, creating and following budgets, and participating in educational or vocational programs Responds to and manages crisis or emergency situations that may arise Accurately completes case notes in a timely manner Manages program participant’s housing chart and ensures all necessary regulatory documentation is current Participates in weekly staff meetings and case conferences Carries out duties essential to the above stated responsibilities as assigned Attends trainings as assigned to increase knowledge of homelessness and substance use disorders Participates in community wide meetings to build a network of other support service workers and to increase knowledge of local resources Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, program participants, and external partners Practices Housing First and Harm Reduction in all duties and interactions with program participants Regular attendance at work location Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others. Requirements

  • Master’s Degree in Human Services or related field OR
  • Bachelor’s Degree in Human Services or related field and a minimum of 6 months human services experience OR
  • Associate’s Degree in Human Services or related field and a minimum of one year human services experience
  • In addition the qualifications above, a valid New York State driver’s license is required and a reliable automobile
