Supervisor- Care Coordination Good Samaritan University Hospital is a 537- bed (including 100 nursing home beds) teaching hospital and a member of Catholic Health Services of Long Island. Job Details • Ensures patient care coordination plans and activities comply with established policies and procedures, standards of care, regulatory requirements, budgetary parameters and strategic objectives. • Supports and fosters a commitment to continuous quality improvement by providing a coordinated system for evaluating patient care outcomes, quality measures, and Key Performance Indicator (KPI) performance. • In full support of System Aim, collaborates with staff and co-leaders to implement Care Coordination Model--roles, future state standard operating procedures (SOPs), SNAP and Escalation Huddles, and Operational Rounding. • Directs all aspects of the Care Coordination Model to improve the Length of Stay performance and patient throughput of the organization, to include the following: Throughput and Patient Placement, Administrative Care Management for Revenue Cycle, Care Coordination Center Operations and Leadership, Organizational Metrics/Measures of Success

Requirements

  • Bachelor's Degree in Nursing required; Masters in Healthcare preferred
  • Ability to develop strong and productive working relationships with direct reports and other leaders
  • Ability to teach/educate and motivate others
  • Ability to form strong team bonds and enhance team performance
  • Strong organizational and quality management skills with ability to handle multiple, competing tasks and priorities
  • Ability to cope with rapidly changing information in a fast paced environment
  • Minimum 3 years of progressive, responsible healthcare management; strong communication and interpersonal skills with ability to work effectively and collaboratively with nursing colleagues, physicians, administrators and entire healthcare team.
