Adapt Integrated Health Care is looking for a qualified ACT Supervisor to join their team in Roseburg, OR! The ACT Supervisor provides direction and clinical oversight to ACT team staff as providing at least 50% direct service to ACT clients. The ACT supervisor provides integrated outpatient mental health treatment services that are based on medical necessity and that support and promote mental health and recovery. ACT staff endeavor to a) Promote recovery, resiliency, wellness, independence and safety for individuals receiving addictions and mental health services and supports; b) Specify standards for services and supports that are person-directed, client-guided, culturally competent, and trauma-informed and wellness informed; and c) Promote functional and rehabilitative outcomes for individuals throughout a continuum of care that is developmentally appropriate. ACT staff responsibilities are to engage and respond in crisis intervention; facilitate access for mental health treatment at Adapt; provide therapeutic interventions including informed consent, advocacy, and integrated care; assist with access to community resources; facilitate development and retention of natural and community supports; provide appropriate individual, group, and community psycho-education; actively work to reduce stigma, barriers, and to build a community foundation for success. Many of the services take place in the community setting where the client lives or at hospitals, jails, shelters, or other community agencies. ACT Supervisor's accomplish these responsibilities through assessment and prescription of treatment services that support rehabilitation, safety, and recovery through measurable outcomes in collaboration with clientele, and their natural supports when appropriate. Some of the interventions provided include comprehensive assessment; collaboratively developed Individual Service and Support Plan; individual and group therapy and skills training and development; case management; referrals, consultation, coordination and collaboration with community partners and providers; Discharge Summary and continuity of care through transfer and referral. ACT Supervisor as well as other treatment providers at Adapt endeavor to support and promote positive outcomes through use of Evidenced-Based, Best Practices whenever possible.
