About The Position

Provide individual counseling, group counseling and case management services to individuals with Substance Use Disorders (SUD) so they may achieve sustained remission from SUD, restore and increase healthy social connections, obtain stable income and permanent housing, and reduce their risk of homelessness. ESSENTIAL FUNCTIONS:• Provide individual counseling, outpatient group counseling, intensive outpatient group counseling, and case management to homeless individuals with substance use disorders.• Provide individual counseling and case management to a case load of program participants.• Facilitate Outpatient Group Counseling and Intensive Outpatient Group Counseling. • Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record.• Establish and maintain regular collaborative relationships with social service organizations and programs that residents may utilize for employment, housing, continuing recovery and other needs. • Participate in daily team meetings, quality assurance meetings, in-service trainings and coordination meetings.• Coordinate with Clinical Director on all critical clinical issues.• Conduct Diagnostic Assessments.• Other duties as assigned.

Requirements

  • High school diploma required - Associates Degree or Bachelor's Degree based on license and experience level.
  • Must have at least 2 years' experience in SUD Counseling or Social Work.
  • CPR and First Aid certification and Safety training required within 30 days of hire.
  • Based on license and experience level; CDCA (Chemical Dependency Counselor Assistant), CDCAII, LCDC II (Licensed Chemical Dependency Counselor II), SWA (Social Work Assistant) LCDCIII (Licensed Chemical Dependency Counselor III), LSW (Licensed Social Worker), LPC (Licensed Professional Counselor).
  • Must be free from sanctions or exclusions from the Office of the Inspector General (OIG) and the State of Ohio Department of Medicaid.
  • Proven ability to meet minimum productivity requirements.
  • Must have successful experience facilitating SUD group counseling.
  • Must have extensive knowledge of Substance Use Disorders and other issues affecting the homeless population.
  • Must have basic computer skills, using Microsoft Office applications.
  • Should have working knowledge of OHMAS, CARF and VA program compliance standards.
  • Demonstrate commitment to the mission, cause of the YMCA, and uphold its values and ethics.
  • Ability to work effectively work with people of different backgrounds, abilities, opinions and perceptions
  • Willingness and ability to be a mandated reporter for any signs or suspicion of child abuse and neglect. All employees are required to follow the child abuse and neglect reporting process.
