About The Position

Provide individual counseling, group counseling and case management services to individuals with Substance Use Disorders (SUD) so they may achieve sustained remission from SUD, restore and increase healthy social connections, obtain stable income and permanent housing, and reduce their risk of homelessness. As a Level III Case Counselor also conduct diagnostic assessments, participate in Quality Assurance activities, supervise interns, provide Level II Case Counselors with mentoring and support, and provide group supervision to clinical staff as needed. ESSENTIAL FUNCTIONS: Provide individual counseling, outpatient group counseling, intensive outpatient group counseling, and case management to homeless individuals with substance use disorders. Provide individual counseling and case management to a case load of program participants. Facilitate Outpatient Group Counseling and Intensive Outpatient Group Counseling. Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record. Establish and maintain regular collaborative relationships with social service organizations and programs that residents may utilize for employment, housing, continuing recovery and other needs. Participate in daily team meetings, quality assurance meetings, in-service trainings and coordination meetings. Coordinate with Clinical Director on all critical clinical issues. Conduct Diagnostic Assessments. Provide mentoring and support to Level II Case Counselors. Conduct quality assurance activities. Participate on Quality Assurance Committee Supervise Interns Provide group supervision to clinical staff, as needed. Other duties as assigned.

Requirements

• Master’s Degree or enrolled in Masters program in relevant field Required
• Must have at least 3 years’ experience in SUD Counseling.
• Minimum License: LCDCIII (Licensed Chemical Dependency Counselor III), LSW (Licensed Social Worker), LPC (Licensed Professional Counselor).
• CPR and First Aid certification and Safety training required within 30 days of hire.
• Must be free from sanctions or exclusions from the Office of the Inspector General (OIG) and the State of Ohio Department of Medicaid.
• Proven ability to meet minimum productivity requirements.
• Must have successful experience facilitating SUD group counseling.
• Must have extensive knowledge of Substance Use Disorders and other issues affecting the homeless population.
• Must have working knowledge of OHMAS, CARF and VA program compliance standards.
• Demonstrate commitment to the mission, cause of the YMCA, and uphold its values and ethics.
• Ability to work effectively work with people of different backgrounds, abilities, opinions and perceptions
• Willingness and ability to be a mandated reporter for any signs or suspicion of child abuse and neglect.
