Provide individual counseling, group counseling and case management services to individuals with Substance Use Disorders (SUD) so they may achieve sustained remission from SUD, restore and increase healthy social connections, obtain stable income and permanent housing, and reduce their risk of homelessness. As a Level II Case Counselor also conduct diagnostic assessments, review and sign off on Level I Case Counselors notes, and provide them with mentoring and support. ESSENTIAL FUNCTIONS: Provide individual counseling, outpatient group counseling, intensive outpatient group counseling, and case management to homeless individuals with substance use disorders. Provide individual counseling and case management to a case load of program participants. Facilitate Outpatient Group Counseling and Intensive Outpatient Group Counseling. Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record. Establish and maintain regular collaborative relationships with social service organizations and programs that residents may utilize for employment, housing, continuing recovery and other needs. Participate in daily team meetings, quality assurance meetings, in-service trainings and coordination meetings. Coordinate with Clinical Director on all critical clinical issues. Conduct Diagnostic Assessments. Review and sign off on Level I Case Counselor Notes. Provide mentoring to Level I Case Counselors. Other duties as assigned.
