About The Position

Healthy Learners is a Ministry of the Sisters of Charity Health System, a statewide organization of regional centers in the Low Country, Pee Dee, Midlands, and the Upstate. Healthy Learners is recruiting for a full-time Student Services Coordinator in Columbia, SC. The Student Services Coordinator will assist the Community Manager with administrative responsibilities and serve as a link between the student client and the health care providers but also interacts with parents and the school personnel, especially the nurse, in the process. The Student Services Coordinator will work with each of these constituencies in order to successfully implement the program. Under the direction of the Community Manager, the Student Services Coordinator will also assist with programmatic responsibilities on an as needed basis.

Requirements

• Experience working in a related professional environment required.
• Enjoys working with children.
• Possesses a current license to operate a vehicle in South Carolina as well as a safe driving record and no evidence of offenses involving children.
• Has knowledge of the geographic areas Healthy Learners serves.
• Ability to work and interact well with others.
• Demonstrated ability to expand own skills.
• Ability to work independently.
• Ability to prioritize assignments.
• Solid organizational and time management skills.
• Computer literate with knowledge of computer software programs being used.
• Will be required to undergo an extensive background check prior to employment.

Nice To Haves

• A Four-year college or university degree in Social Services, Health Care, or an appropriate discipline preferred.

Responsibilities

• Serves as a primary person for safely transporting children to provider appointments.
• Delivers glasses, medications and other resources to the student or school nurse in a timely and responsible manner.
• Appropriately documents care coordination activities which include student medical appointments and follow up care as specified by the provider along with Healthy Learners intervention and plan.
• Positively communicates and represents the Healthy Learners mission and vision.
• Serves as a positive caring role model for students as evidenced by actions and behaviors.
• Participates with other members of the staff in fulfilling the goals and objectives of Healthy Learners.
• Assists with secondary office operations as assigned.
• May be a representative of and spokesperson for the program to any of the relevant constituencies.
• Other duties as assigned.
