About The Position

The Street Outreach Services Coordinator is responsible for enhancing and coordinating professional street outreach services for individuals experiencing homelessness in Indianapolis. This role provides leadership and direct supervision to street outreach staff, ensuring services are delivered in alignment with Horizon House policies, Professional Blended Street Outreach (PBSO) standards of practice, ethical guidelines, and funder requirements. The Coordinator works collaboratively with the Outreach Manager, internal program leaders, and community partners to support consistent, high-quality outreach, coordinated entry, and continuity of care. This position reports to the Street Outreach Manager. Please note that this is a grant-funded, at-will position. While this role is intended to be ongoing, its continuation is subject to the periodic renewal of grant resources.

Requirements

  • Knowledge of homelessness, chronic medical and mental health conditions, substance use disorders, and evidence-based approaches to housing stability. Strong commitment to best practice models, ethical decision-making, and measurable outcomes
  • Demonstrated ability to supervise, coach, and support staff in a field-based, team-oriented environment. Ability to manage group dynamics, inspire teamwork, and promote professional development
  • Skilled in building and maintaining collaborative relationships with community partners and system stakeholders. Ability to represent the program effectively in meetings, case conferences, and cross-agency coordination
  • Collects, analyzes, and applies program data to inform strategies and improve services. Ability to maintain timely, accurate electronic documentation.
  • Strong verbal, written, and interpersonal communication skills, including crisis de-escalation and mediation. Able to clearly convey expectations, procedures, and service information to staff, volunteers, and clients.
  • Plans and attends to details and pursues quality in accomplishing multiple tasks in an organized and timely fashion. Exhibits careful attention to detail and follow-through.
  • Adheres to procedures and protocol, including maintaining confidentiality and adherence to organizational values. Uses discretion when sharing and protects private information.
  • Proficiency with G Suite, Microsoft Office, databases, and willingness to learn agency management systems.
  • All candidates must pass a criminal background check.
  • Must have a valid driver’s license, up-to-date auto insurance, and a good driving record.
  • Ability to occasionally work a flexible schedule beyond normal business hours.

Responsibilities

  • Provide direct supervision, orientation, training, coaching, and ongoing support to street outreach team members, ensuring services are delivered consistently and in alignment with best practice models.
  • Participate in recruitment, interviewing, hiring, onboarding, scheduling, and performance management of outreach staff, including performance evaluations and timely resolution of concerns.
  • Partner with outreach team members to conduct professional street outreach in Indianapolis as needed to support engagement, safety, and service continuity.
  • Ensure accurate, timely documentation of client services, assessments, outreach contacts, and outcomes in HMIS and other required systems.
  • Complete Coordinated Entry System (CES) assessments as needed and represent the outreach team at CES case conferences in accordance with Indianapolis Continuum of Care (CoC) guidelines.
  • Follow up on referrals and coordinate with other outreach teams and service providers to promote continuity of care and reduce service duplication.
  • Support implementation and continuous improvement of service delivery processes to maintain high-quality, ethical, and measurable outreach and case management practices.
  • Establish and maintain positive, productive working relationships with community partners, funders, and system stakeholders.
  • Support PIT Count planning through the Outreach Manager and participate in PIT Count activities as assigned.
  • Assist with program evaluation, internal compliance monitoring, and preparation for funder or system reporting as needed.
  • Model and enforce ethical conduct, confidentiality, cultural sensitivity, and respect for client self-determination.
  • Support staff in assessing and responding to safety concerns, crises, and potential hazards using trauma-informed, de-escalation, and problem-solving approaches.
  • Attend and contribute to internal meetings, trainings, and community-based collaborations.
