Start of Care Physical Therapist
About The Position
We’re expanding our team and looking for a Start of Care Physical Therapist to join our South Hills Home Health team! In this role, you’ll be the first clinician our patients meet, performing Start of Care evaluations, creating individualized care plans, and coordinating with our home health team to ensure patients succeed in the comfort of their homes. Your schedule will include up to 7 units per day, covering Start of Care, Resumption of Care, Recertification, Discharge, and skilled PT visits as needed.
