Position Summary Trinity Health At Home is seeking a Speech‑Language Pathologist Clinical Fellow to join our growing home‑based rehabilitation team. This role is ideal for a recent master’s graduate looking to complete their Clinical Fellowship Year with strong mentorship, diverse caseload experience, and the autonomy of providing 1:1 care in a home environment. The Clinical Fellow will assess, diagnose, and treat adult patients with communication, cognitive‑linguistic, and swallowing disorders under the supervision of a certified and licensed CCC‑SLP mentor, in alignment with ASHA requirements. Requirements

  • Master’s degree in Speech‑Language Pathology from an accredited program
  • Eligible for temporary/limited speech‑language pathology license in Michigan
  • Valid driver’s license, reliable transportation, and proof of auto insurance
  • Strong interpersonal communication, organization, and problem‑solving skills
  • Passion for home‑based care and serving diverse populations
  • Ability to work independently with support from a mentor and clinical team

Nice To Haves

  • Previous clinical practicum experience with adult or geriatric populations
  • Interest in neurological rehabilitation, dysphagia management, and cognitive‑linguistic treatment

Responsibilities

  • Provide speech, language, cognitive‑communication, and dysphagia assessments and treatments in patient homes
  • Develop individualized treatment plans and communicate progress with patients, caregivers, and interdisciplinary team members
  • Document services and outcomes in accordance with organizational and regulatory standards
  • Participate in interdisciplinary case coordination meetings
  • Maintain professionalism, ethical practice, and patient‑centered communication
  • Successfully meet all ASHA CFY competencies under the guidance of an approved CCC‑SLP mentor
