About The Position

As a Specialist, Lifesharing Services, you’ll support UCP Central PA’s mission by coordinating and overseeing lifesharing placements that promote independence, safety, and meaningful community living. You’ll partner with individuals, families, and caregivers to develop and monitor support plans, ensure regulatory compliance, and provide ongoing training and guidance, helping create stable, supportive home environments for the people we serve

Requirements

  • 18 years of age
  • A valid driver’s license with at least 6 months of driving experience
  • Access to a vehicle with valid insurance, registration, and inspection
  • High School Diploma or GED required
  • 2–5 years of related experience required; experience in human services strongly preferred
  • Ability to communicate, read, write, and interpret English
  • Strong organizational, planning, and communication skills with attention to detail
  • Ability to build relationships, work collaboratively, and maintain confidentiality

Nice To Haves

  • Associate’s degree or technical certification within a related area preferred
  • Preferred knowledge of individuals with intellectual and developmental disabilities, autism, and/or the aging population
  • Preferred experience with Medicaid-funded services and PA regulations (Chapters 6100/6500)
  • Preferred familiarity with human service program operations, compliance, and documentation systems
  • Preferred background in psychology, education, social work, healthcare administration, or related field

Responsibilities

  • Evaluate referrals and coordinate lifesharing placements, ensuring a strong match between individuals and caregivers
  • Complete home studies and review caregiver applications to ensure safe, compliant, and supportive living environments
  • Develop and monitor Individual Support Plans (ISPs), collaborating with families and team members to meet individual needs
  • Conduct regular home visits and ongoing oversight to ensure quality care, compliance, and positive outcomes
  • Train and support caregivers on person-centered practices, safety, and regulatory requirements
  • Maintain accurate documentation and complete assessments, reports, and audits within required timeframes
  • Collaborate with families, Supports Coordinators, and external partners to coordinate services and support individuals in the community
