About The Position

Provide support and collaboration with the special populations team by assisting with the coordination of program activities and day to day operations. Provide case management services to program participants. ----- Provide support and collaboration with the LGBTQI+ team by assisting with the coordination of program activities and day to day operations. Collaborate with Primary Care providers and the team in assessing, planning, coordinating, and managing the biopsychosocial needs of the individuals for care services and patients. Deliver support services to patients to improve patient care for the LGBTQI+ TGNCNB population. Provide case management services and utilize culturally appropriate material. Support the process of the multi-disciplinary collaborative patient-focused care; ensure the patient receives timely and unimpeded access to the appropriate, high-quality, cost-effective health care and social needs of the patients. Program functions may include screening, intakes, implementation and coordination of services, monitoring, and discharge/closure. Coordinate and implement evidence-based interventions; monitors program and patient data to determine effectiveness of quality improvement projects for participants. Determine patient needs and social determinants of health and refer to programs to assist the patient. Document and track all referrals. Document all services in the patient EMR to ensure accurate data compilation for AIRS reporting and any other required reporting system. Attend training, continuing education, conferences, and webinars to stay current on education and techniques, interventions, and best practices used with LGBTQI+ TGNCNB patients. Demonstrate understanding & strict adherence to laws & policies related to HIV Confidentiality law, HIPPA, and disclosure of health-related information. Participate in Quality Improvement projects.
