About The Position

Under the direct supervision of the Director of Clinical Services (DOCS), the County Nurse Manager (CNM) is responsible for the day-to-day effective and efficient operation of the assigned practice’s clinical operations. The CNM ensures that staffing needs are met, supplies are available and accessible to the care team and that quality patient care and customer satisfaction remain central to the activities of clinical employees at the practice site. Core job duties are operational, clinical, clerical and supervisory. Under the supervision of the DOCS, the CNM assists to facilitate patient care in the clinical environment. S/He is an essential part of the care team from the onset of the visit until its completion. The CNM will be involved in both the clinical and administrative areas. The CNM is responsible for ensuring all MAs are capturing and documenting clinical and demographic information accurately in the Electronic Medical Record (EMR). S/He performs as training and vaccination training as needed to staff. The CNM also coordinates and assists with purchasing and management of medical supplies as directed by DOCS. S/He communicates directly with the DOCS for direction and information for clinical flow and procedures.

  • Certification(s)/Licensure: Current NC nursing license

Responsibilities

  • Devotes a minimum of 50% of time to direct patient care.
  • Devotes up to 50% of time to administrative duties.
  • Assists with overseeing the day-to-day clinical functions of all CRCHC assigned clinics within his or her county
  • Supervises and evaluates designated CMA’s, LPN’s and other assigned clinical support staff
  • Confers with the Human Resources Manager in the selection, promotion, discipline, and discharge of direct reports.
  • Meets regularly with clinical staff to receive input and share information.
  • Assist with telephone triage or follow up with patients as directed by the provider
  • Under the direction of the provider, assists with patient health education and follow up on established patient plan(s) of care.
  • Complete in a timely manner, the assigned clinic maintenance tasks including but not limited to the following: setting up instruments and equipment including a working knowledge of all equipment and the ability to train other personnel on its use. Running test and controls on equipment and the ability to teach other personnel. Troubleshooting breakdowns, maintaining supplies, performing preventative maintenance and calling for repairs. Ensuring clinic is ready for next day operations.
  • Fulfill clerical responsibilities as assigned including but not limited to the following: obtaining lab/x-ray reports, hospital notes and referral information.
  • Assist provider with various procedures and accurately document results of completed procedures. The ability to train other personnel on appropriate procedure protocols.
  • Assist the DOCS in assuring supplies are ready and that inventory of supplies is maintained to meet the needs of the clinic and provide support to another clinic as needed.
  • Maintain safe, secure, and healthy work environment by following and enforcing established policies and procedures and the ability to professionally encourage others to do the same to support a healthy team atmosphere.
  • Maintain patient confidentiality and protects operations by keeping protected health information confidential and secure.
  • Participates in mandatory in-services and drills, attends staff meetings and other trainings, including appropriate age specific educational trainings offered on regular basis for professional development.
  • Maintain strict adherence to infection control guidelines established by the organization.
  • Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services. Includes vaccine records, calling and notification to parents/guardians of vaccines needing updates. Includes brick and mortar abnormal pap smear management for his or her county which must be performed monthly. Responsible for maintenance of the STD binder within all clinics of his or her county, which must be updated every 3 years.
  • Support current incentive, regulatory and certification requirements (such as Meaningful Use, PCMH and UDS) through documentation, participation in initiatives, and other activities as directed.
  • Assist the DOCS and Vaccine Manager in the maintenance of vaccine record and vaccine inventory, assuring vaccines are documented accurately and that stock is maintained to meet clinic population needs for both State Vaccines and Private Stock. Removes inventory for vaccine procedures in the absence of the DOCS to assure accuracy of delivery to patients. Completes all requirements and education dictated by the NCIR guidelines for administrator back up. Assist as directed by Vaccine Manager for reports, follow up records, and notifications. Is part of the response team for power outages at the direction of the Vaccine Manager to assist with transportation of vaccines as needed.
  • Maintains lead in the training and onboarding of new employees to the clinical setting and gives feedback of needs and training opportunities to the DOCS. Also, maintains lead in conducting an annual or as needed clinical skills assessment training for clinical support staff.
  • Oversees the certifications for the MAs and LPNs are in compliance and assisting with recertification of RMA, CCMA, or BLS certification in coordination with Human Resources. Also, oversees Relias compliance of clinic staff within all of his or her county locations.
  • Performs staff evaluations for the MAs, Clinical Supervisors, and LPNs within his or her county.
  • Assist the DOCS in ensuring the department is ready for the start of clinic and ensures that each clinic closes the day.
  • Assists DOCS in ensuring the referral department closes the gaps of care within a timely fashion.
  • Responsible for ensuring CMA coverage at each location within his or her county and ensuring coverage across CRCHC network of clinics.
  • Responsible for ensuring coverage of CMA in conjunction with CRCHC’s other County Nurse Manager to ensure coverage through CRCHC network of clinics through PTO, Callouts, etc.
  • Participates in all meetings needed with the DOCS (i.e. Admin, Operations, QI, Etc.)
  • Assist in the coordination of clinical department meetings and communications that go out to department in regards to company wide announcements, policy or protocol updates, staff education, etc.
  • Handle administrative matters and ensures that all program /project activities are implemented in accordance with established policies and procedures as well as grant requirements.
  • Participates in assisting with coordinating, management and purchasing of medical supplies and equipment as directed by the DOCS.
  • Rounds to all clinics within his or her county and discuss issues with the clinic staff and assists the DOCS in problem-solving for those issues.
  • Responsible for submitting a morning report documenting the clinical flow for the day including late/early clinic and call outs within his or her county.
  • Responsible for writing and submitting incident reports for any occurrence that affects patient care within his or her county.
  • Participate in QI meetings under the direction of the DOCS, which can include HTN/DM meeting, Colorectal Collaboration, Patient Cycle Time Committee, etc.
  • Perform other duties as assigned.
