Provide Peer support services to program participants, conduct educational and support groups, and plan and implement outreach events. ----- Provide support and collaborate with the LGBTQI+ team by assisting with the coordination of program activities and day to day operations. Collaborate with Primary Care providers and the team in assessing, planning, coordinating, and managing the biopsychosocial needs of individuals for care services and patients. Support the case manager in delivering support services to patients to improve patient care for the LGBTQI+ population. Work with the case manager to coordinate and provide case management services to program participants and assist with appointments, client outreach calls, and utilize lived experience to assist patients as they move through their care and treatment process. Document all services in the patient EMR. Create and implement consumer feedback activities and participate in quality projects. Provide activities that raise awareness of and promote access to services that support relationship and family building, prevention, and support services for the TGNCNB individuals who have experienced discrimination, bias crimes, and domestic violence. Assess for and provide SDOH referrals that will support the needs of the TGNCNB individual. Provide activities that address and promote access to health, wellness, and human services for TGNCNB individuals. Provide educational activities regarding TGNCNB access to healthcare. Maintain and update social media accounts with pertinent program and educational information. Provide educational and support and focus groups, education to providers, and the general population virtually and in person. Plan, prepare, and or attend community events. Attend training, continuing education, conferences, and webinars to stay current on education and techniques, interventions, and best practices used with LGBTQI+ TGNCNB patients. Demonstrate understanding & strict adherence to laws & policies related to HIV Confidentiality law, HIPPA, and disclosure of health-related information.
