About The Position

Family Service League is seeking a full-time Social Worker for the High Utilization Referral program. The Social Worker engages, assesses, and provides linkages to referrals made by law enforcement through the High Utilizer Referral Program. They provide in person and telephonic crisis intervention, de-escalation and assessment as part of Mobile Crisis. Requirements

  • Master’s Degree required.
  • LMSW, LMHC, LMFT or related licensed clinical degree required.
  • Minimum of 6 months of experience in a behavioral health setting required. Clinical experience preferred.
  • Proficient computer skills, including Microsoft Office required. Experience with an Electronic Health Record preferred.
  • Excellent interpersonal and verbal and written communication skills required.
  • Ability to problem solve, multitask, make clear decisions and work as part of a team required.
  • Valid and clean NYS Driver’s License and ability to safely operate a motor vehicle is required for the Mobile Response Team.

Nice To Haves

  • Bilingual in Spanish preferred.
  • Evening, weekend, and overnight hours may be required.

Responsibilities

  • The Social Worker will triage all cases referred into the High Utilizer Referral Program.
  • Engage and build rapport with individuals and their supports via in person and telephonic outreach.
  • Provide in person and telephonic crisis intervention and de-escalation.
  • Conduct clinical assessments, provide referrals, and follow up on linkages
  • The Social Worker will monitor individuals’ utilization of crisis services and community supports vs ongoing engagement with law enforcement.
  • Utilize resources such as PSYCKES to review relevant clinical information.
  • Work closely with law enforcement to accept referrals and provide a co-response to individuals in crisis, when clinically necessary.
  • The Social Worker will complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record.
  • Coordinate with outside providers and collateral contacts.
  • Attend routine supervisions and team meetings.
  • Work as part of a multidisciplinary team, conferencing cases, communicating outcomes and providing follow up.
  • Complete trainings and maintain proficiency with de-escalation and crisis intervention skills.
  • All other duties as assigned.
