We are currently seeking a Full-Time Licensed Social Worker to join our Manhattan team. This rewarding role offers the opportunity to make a meaningful impact by helping individuals and families age safely, comfortably, and with dignity in the comfort of their own homes.
In this position, the Licensed Social Worker will deliver comprehensive, client-centered support by addressing psychosocial needs and ensuring access to essential resources. The role includes conducting client and family meetings, completing assessments, developing individualized care plans, and coordinating community-based services that promote independence, stability, and overall quality of life.
This is a full-time position with an annual salary range of $70,000–$80,000, based on experience.

Requirements

• Master’s degree in Social Work (MSW).
• Active New York State LMSW or LCSW license.
• Minimum of one year of social work experience in a healthcare setting; home care experience preferred.
• Experience creating individualized service plans and coordinating community-based resources.
• Demonstrated ability to provide compassionate, client-centered care.
• Strong communication and interpersonal skills.
• Excellent organizational skills with the ability to manage caseloads and maintain accurate documentation.

Responsibilities

• Conduct thorough psychosocial assessments of clients and families, evaluating social work needs and living environments through professional observation and interviewing techniques.
• Act as the primary non-clinical point of contact for clients and families, leading meetings and facilitating ongoing communication.
• Partner with clients and families to identify needs and connect them with appropriate community resources and services.
• Perform home visits to ensure safe, supportive, and stable home environments.
• Provide social work services in alignment with Affirmed Home Care policies, procedures, and the Standards of Social Work Practice.
• Collaborate with interdisciplinary team members to develop, implement, and regularly review individualized treatment plans.
• Educate clients and families on care plans, available resources, and best practices to support ongoing care.
• Identify opportunities to enhance the home environment, promote independence, and support successful aging in place.
