Responsibilities: The social worker is responsible for providing clinical services to children ages 0-21 who are patients of DBP or have been referred to DBP. Services will be provided utilizing a multidisciplinary, continuity of care model. Other general responsibilities include manpower training, technical assistance to agencies and programs and research. Consultation to internal and external programs related to IDD. Strong knowledge of IDD, OPWDD and the educational system.

Requirements

Must hold a Master of Social Work degree from an accredited institution.
NYS licensure required within 18 months of employment

Responsibilities

Provides SWK treatment, referral and advocacy to patients who need case coordination during periods of stress and serves as a consultant to patients and families regarding community resources, financial assistance and special programs (waivers, Health homes, OPWDD and the educational system)
Conducts phone screens and intakes which involves clinical assessments with independent skills to provide interventions and referrals. This involves: advocacy, coordination and referrals to internal and external programs.
Completes a psychosocial assessment to identify barriers to care and programs to support their child’s developmental disability and transitions through the lifespan.
Documents SWK activity in patients’ medical record according to SWK division policy.
Identifies and manages cases of suspected child abuse and neglect in the DBP Center
Collaborates with other social workers and members of the health care team to ensure continuity of SWK care between clinical settings as well as to achieve well-coordinated, quality care.
Coordinates with other health, developmental, educational and social service agencies involved in the care and support of patient and family.
Provides social work consultation on pediatric patients with IDD.
Provides Social Work participation on the following DBP teams and Hospital committees Hospital IDD task force EPIC Behavioral Plan Committee CHIRP- OPWDD intensive behavioral program, which involves a high level of understanding of behavioral interventions. Feeding- Which involves an understanding of feeding disorders and interventions. Behavioral Programs at DBP – BIFF and BTP- which include a strong knowledge of the evidence-based programs and developmental disabilities.
Participates and presents in State wide training through ECHO and presentations through national Autism Organizations.
Serves as an ECHO Hub Team Member
Offers presentations to community agencies through presentations and 1:1 communication
Part of the interdisciplinary team working with trainees from multiple disciplines (MD, NP, RN, OT, PT and SW).
Identifies risk management issues and reports them to SWK administration
Provides back up coverage for other social worker as needed
Other duties as assigned- May provide field work instructions and supervision to SWK students
Participates in activities in the SWK Division including; absence approval, time reporting and other mandatory trainings.
Participate in administrative activities in DBP, may include staff meetings and program development
