Performs psychosocial assessments that are complete and timely: they identify major factors affecting care plan and coping and are completed within 5 days of admission. Develops care plans that are appropriate to meet the needs of individual patients and families and are carried out or revised appropriately. Solves routine care plan problems independently as appropriate. Advocates effectively for services from community organizations. Effectively elicits emotional expression in short-term counseling with patients and families. Meets established goals in a timely manner. Represents the organization's social work department in internal and external meetings, presenting accurate information effectively. Records planned and completed visits, phone calls and meetings in NDoc and other required media in a timely manner. Provides social work counseling to patient and family, with appropriate follow-up visit or phone call to ensure goals are met. Participates in interdisciplinary team meetings and/or individualized conferences. Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations. Participates in all required in service education programs and meetings. Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance. Performs other duties as assigned.
