About The Position

Provides case management to coordinate with internal and external ancillary services for members and families. Assists with members as needed (ie: hygiene, meals, ambulation, arrivals and departures, and social activities). Participates and performs assessments, care plan meetings, and annual PCP meetings. Maintains formal written communication with members and families and facilitates counseling through support groups and one-on-one sessions. Maintains accurate files and records of member and social services information.Participates in community outreach programs, including communication with other health and social service providers.

Requirements

  • Certified Social Worker (CSW) with a Bachelor’s degree in Social Work, OR a Licensed Social Worker (LSW) with a Master’s degree in Social Work required.
  • Bilingual in English and Spanish required.
  • Minimum one (1) year social work experience in a health care facility, preferably working with seniors and/or adults with disabilities strongly preferred.
  • Knowledge of applicable state and federal environmental and OSHA regulations
  • Extremely strong collaborate skills across all levels of staff and thrives in a collaborative and team environment.
  • Physical requirements to include but not limited to: Push, pull, squat, turn, pivot, transfer and lift at least 50 pounds. Sit, Stand and/or Walk for extended period of time while providing physical assistance to members
