The social worker acts as a patient advocate to the hospital clients. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost efficient patient outcomes. Determines medical and social services eligibility on new admissions. Interviews clients to assess client’s social situation; identifies psycho-social problems that would interfere with treatment plan, provides supportive counseling to clients, families and staff members; assists them in understanding client’s physical and/or mental disability; encourages and motivates patient and family to actively participate and cooperate in treatment plan. Conducts home visits for follow-ups or home assessments to ensure client’s treatment plan will be successful. Coordinates with other agencies and organizations in the provision of services for clients or family. Works collaboratively with interdisciplinary staff internal and external organizations. Participates in quality improvements and evaluation processes related to the management of patient care. The social worker is on-site and available seven days a week and during holidays, therefore, is required to work an on-call rotation schedule.
