About The Position

Baptist Health & Rehab, a Cassia community, is hiring a Social Worker or Social Services Designee to join our compassionate care team. As a Social Worker or Social Services Designee, you will provide emotional support and advocacy for residents and families, assist with care planning, and help address psychosocial needs throughout the resident experience. You’ll work closely with the interdisciplinary team to ensure smooth admissions, ongoing support, and effective discharge planning. The ideal candidate is organized, approachable, and committed to enhancing quality of life for the residents we serve.

Requirements

  • Licensed Social Worker preferred; however, candidates without a degree will be considered for a Social Worker Designee role
  • Experience working in LTC preferred.
  • Must have a strong social service background.
  • Must have knowledge of state and federal guidelines related to long-term care and resident rights.
  • Excellent customer service.
  • Team player and self-starter.
  • Successful candidates will be mission-minded with excellent communication skills.

Responsibilities

  • Provide social services support to approximately 140 residents and their families addressing medical, emotional, physical, and psychosocial needs.
  • Assist with MDS processes, including BIMS & PHQ-9.
  • Coordinate social services across the facility, including individualized plans of care, counseling, follow-up services, and behavioral or psychiatric support plans.
  • Act as a liaison and advocate for residents, ensuring basic human and resident rights are upheld and maintained.
  • Participate in admissions, discharge planning, and appropriate referrals to support smooth transitions of care.
  • Provide resources and referrals to residents, families, and staff as needed.
  • Collaborate closely with the interdisciplinary team, including working in conjunction with two full-time Social Workers.
  • Provide PRN coverage for Social Workers during vacation or planned absences.
  • Assist residents with adjustment to the community, including conducting tours and orientation support.
  • Demonstrate a strong commitment to compassion, excellence, and quality care.
