About The Position

The Social Services Designee supports the Social Services Director in planning, developing, organizing, implementing, and directing the social services programs within the facility. This role ensures that the emotional, social, and psychosocial needs of residents are identified and met in accordance with regulatory requirements and facility standards. The Social Services Designee will assist residents with concerns related to their rights, living wills, and other supportive needs. This position includes regular interaction with residents and families, providing non‑medical progress updates, supporting adjustment to levels of care, and coordinating with community agencies as needed. Responsibilities

  • Conduct comprehensive psychosocial interviews and assessments for residents and families.
  • Develop individualized care plans addressing psychosocial needs.
  • Assist with scheduling, preparing for, and facilitating resident care conferences.
  • Assist residents and families in adjusting to levels of care and provide ongoing support.
  • Educate residents/families on healthcare levels, community resources, healthcare changes, and discharge planning.
  • Discuss advance directives and document resident wishes.
  • Promote effective communication and collaboration among healthcare team members.
  • Maintain routine contact with residents and families, including non‑medical progress updates.
  • Assist in reviewing resident complaints and concerns, ensuring appropriate follow‑up.
  • Complete daily documentation of interactions and services in accordance with facility, state, and federal regulations.
  • Document all discharge planning efforts and coordination with outside agencies.
  • Complete MDS assessments and follow RAI guidelines.
  • Maintain accurate records of changes in condition, goals, discharge plans, and participation in care planning.
  • Coordinate referrals to community agencies and social/health organizations when specialized support is needed.
  • Assist residents in obtaining durable medical equipment as appropriate.
  • Network with community partners to support resident needs.
