Requirements
- Must obtain a BA Degree in Social Work, Psychology, or a related field.
- Social Services Designee Certificate required.
- Three years of experience in a skilled nursing facility, mental health, or related fields.
- Demonstrated ability to assess client needs, implement treatment strategies, and provide individual, group, and family counseling and education.
Responsibilities
- The Social Services Designee is responsible for the ongoing assessment of each resident’s social service needs.
- Will assist with the development and implementation of a plan of care to meet identified needs.
- Responsible for providing medically related Social Services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident.
- The Social Service Designee will function as a participating member of the IDT (Interdisciplinary Team).
