The Social Services Designee is responsible for the ongoing assessment of each resident’s social service needs. Will assist with the development and implementation of a plan of care to meet identified needs. Responsible for providing medically related Social Services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident. The Social Service Designee will function as a participating member of the IDT (Interdisciplinary Team). The Gardens at Idylwood is a 66-bed nursing and rehabilitation building on the Idylwood Care Center campus. The Gardens at Idylwood provides short-term rehabilitation as well as extended stay services. Requirements

Must obtain a BA Degree in Social Work, Psychology, or a related field.
Social Services Designee Certificate required.
Three years of experience in a skilled nursing facility, mental health, or related fields.
Demonstrated ability to assess client needs, implement treatment strategies, and provide individual, group, and family counseling and education.

Responsibilities
