About The Position

We are looking for a Social Worker to support and advocate for adult Skilled Nursing Facility TCU/LTC residents in coping with the psychosocial aspects of their lives, including grief, loss, and change. To coordinate transitions, including room changes, discharge plans, and end-of-life preferences. 72 hours every two weeks Week 1: M-F, Week 2: M-TH 8 am-4:30 pm Full Benefits Package: (Paid Time Off, Retirement monies, Medical, College funds, Wellness reimbursement). Requirements

Bachelor's Degree: List – BSW (from an accredited school of social work)
Licensed by the State of Minnesota Board of Social Work
0-1 years Experience

Nice To Haves

Two years’ experience in LTC or TCU
Experience in Medical Social Work
1-3 years of experience

Responsibilities

Manage resident caseloads
Coordinates admissions with the admission team assigned
Serve as Resident Advocate and Facility Liaison for Family Council as appropriate
Facilitates necessary discharge planning and coordination
Assists in Vulnerable Adult investigation and completes necessary documentation and reports
Perform other duties as assigned and directed
