Social Worker
About The Position
As the season of renewal begins, our long-term care facility is growing! We are dedicated to providing compassionate, resident-centered care that nurtures both physical and emotional well-being. We are currently seeking a Social Worker who brings clinical expertise, empathy, and a collaborative spirit to our interdisciplinary team. The Social Worker will support residents and their families by addressing psychosocial needs, coordinating care plans, and enhancing quality of life. Candidates with an LPN or RN license and a degree in Social Work or related field are highly encouraged to apply, as this role bridges medical care and emotional support.
Requirements
- Bachelor’s degree in Social Work, Psychology, or related field (required)
- Strong communication and interpersonal skills
- Ability to handle sensitive situations with professionalism and compassion
Nice To Haves
- Active LPN or RN license (preferred)
- Experience in long-term care, geriatrics, or behavioral health (preferred)
Responsibilities
- Conduct psychosocial assessments and develop individualized care plans
- Provide counseling, emotional support, and crisis intervention
- Collaborate with nursing staff, physicians, and interdisciplinary teams
- Assist with discharge planning and transitions of care
- Advocate for residents’ rights and well-being
- Support families with education, communication, and resources
- Ensure compliance with state and federal regulations
