About The Position

Social Worker – $45,760/year Maple Crest Health Center | American Baptist Homes of the Midwest (Omaha, NE 68104) Why Join Us? People First: Support residents and families through meaningful emotional and psychosocial care Competitive Pay: Starting at $45,760/year with flexibility based on experience Schedule: Day shift, 9:00am–4:30pm Collaborative Team: Work as part of a strong interdisciplinary care team Mission Driven: Support residents across the full continuum of care guided by ABHM values What You’ll Do: Conduct psychosocial assessments and provide counseling to residents and families Support admissions, transitions, and adjustment to aging or illness Collaborate with the interdisciplinary team on care planning and documentation (including MDS) Assist with discharge planning and coordination of community resources Advocate for residents and provide crisis intervention as needed Maintain accurate documentation and support financial/insurance processes Serve as a liaison with families, agencies, and mental health services

Requirements

  • Licensed Social Worker (per state requirements)
  • At least 1 year of interdisciplinary team experience, with long-term care or hospice preferred
  • Strong communication, critical thinking, and documentation skills
  • Ability to work in a fast-paced, emotionally sensitive environment
  • Proficiency with computer systems and maintaining confidentiality
  • Ability to collaborate effectively with residents, families, and care teams

Nice To Haves
