About The Position

Social Work Services at the Mount Sinai Hospital The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities. This position is in a program that provides medical and mental health monitoring, treatment, and follow-up care to rescue, recovery, restoration, and clean-up workers who responded to Ground Zero following 9/11, many of whom are diagnosed with World Trade Center (WTC)-related physical and mental illnesses. The social worker in this position is responsible for providing case management services to WTC responders who are members of the World Trade Center Health Program at Mount Sinai. The social worker will provide comprehensive screening, counseling, assistance, and advocacy to patients seeking entitlements and other supportive services. This position will assist with the delivery of case management services to patients with complex psychiatric and medical needs. The social worker will be responsible for providing direct, clinical services including remote and in-person consultations to patients.

Requirements

  • NYS LMSW or LCSW.
  • Excellent engagement, assessment, and collaborative skills, with ability to work flexibly in a high-volume, fast-paced setting.
  • Knowledge of public benefits programs and interest in learning about issues specific to population, including occupational safety and health issues, unions, workers’ compensation, and employees’ rights.
  • Exposure to individuals with mental health diagnoses, especially with PTSD, preferred.
  • Minimum one year in current position.

Nice To Haves

  • Experience with unions or worker organizations, geriatric and/or oncology social work desired .
  • Bilingual (English/Spanish or English/Polish) preferred.

Responsibilities

  • Assess individual’s biopsychosocial status, social system, and resources.
  • Assist patient in navigating complex health and social service systems.
  • Monitor patient’s clinical, psychosocial, functional, or mental status and communicate any significant changes to other medical and mental health team members.
  • Assess level of patient need and identify appropriate frequency of contact.
  • Reinforce patient’s support systems/Collaborate with family and caregivers as indicated.
  • Coordinate and develop patient’s care plan with other medical and mental health team members.
  • Collaborate to ensure consistent access to care with WTC Health Program’s network of external mental health providers.
  • Refer patient for necessary services and provide ongoing intervention related to service connections and identified needs.
  • This position is based at the 345 East 102 nd Street clinic and will require the ability to work remotely as needed with access to basic technological equipment (e.g., computer with webcam, high-speed internet, and reliable cellphone service in space established for work) and a secluded area in which confidential work can be done.
  • The ability to work evening/weekend hours may be required.
